I worked 30 years in a general hospital and the phrase “nursing diagnosis” is unfamiliar to me, so I’m not sure what you are asking. A much more complete history would be helpful–has she been paralyzed since the initial infection, or did the muscle weakness return as she aged. I would be very concerned about her pulmonary and swallowing history and any current problems. Is she alert and oriented? A Plan of Care requires much more information than you give. Talking to family, look at records and researching a bit more on Poliomyelitis would be very useful. There is a list of articles you might find helpful at the link below.
If you are having to ask about a Nursing Diagnosis, then I assume you are a student, and then know that a Nursing Diagnosis cannot be based upon a Medical Diagnosis, such as Poliomyelitis.
Instead, a Nursing Diagnosis is based upon the patient’s signs and symptoms, such as the ones you list above. If you don’t have access to a current Nursing Diagnosis book, I recomment you look at:

An example would for, “Personnel must use a lift to move her due to loss of use of her extremeties,” definitely include:
*Mobility Impaired, bed – under Activity/Rest
and could easily entail others, depending on your patient, such as (but not limited to):
*Body Image, disturbed – under Ego/Integrity
*Constipation – IF related to the Polio (you asked specifically about the Polio, but is definitely a current diagnosis of the patient regardless)
*Various Self-Care Deficits – under Hygeine
*Comfort, impaired
*Various Safety deficits
…and so on.
If you *are* a student, then since your instructor will be grading your ND, I recommend you speak with him or her about how s/he expects you to choose/prioritize these diagnoses (you can sometimes find a hundred of them which apply, but most of them aren’t really important to the care you will provide as a nurse), or a tutor, or higher level students, to see what they think about how your program views such selection/prioritization.
This is probably not what you were hoping for, but I believe it is the best you can get from a question like this without the people who answer knowing more about your specific patient or your programs expectations.
