Ha Ha. NCPs are the complete bane of nursing students. Thank god after nursing school they are pre-printed. However, they are a framework guideline, if you will, where you identify a nursing problem and formulate the plans that you would take to provide nursing care to address the problem.
So you would say, get a problem where the patient is a bed-ridden elderly woman & asked to formulate a care plan. One issue (problem) would be (there are many): 1. Potential for skin breakdown; then your care plans would be steps to prevent this or deal with this such as:
a. turn patient frequently b. pad bony prominences c. ambulate if possible d. notify house officer if decubiti noted. Etc etc.
