In my nursing class we had a patient assigned to us at a nursing home and we have to do a care plan on them. I need a little help and advice. Ok, my patient has end stage renal disease. I was going to do this as my care plan but then I had to go back to the nursing home and saw that my patient had CHF. I know CHF has more priority so, now I am having to redo everything.
Ok, so I know I will use Decreased Cardiac Output as my NANDA diagnosis but what should I use for retated to? My patient has diabetes, HTN, pitting edema, seizure disorder, pneumonia.
Also what are some other NANDA diagnoses should I use and can you put them in order for me?
I don’t have a clue what NANDA is, but immobility is one nsg dx,
skin integrity, nutrition and fluid restrictions
multiple medications and safety issues
end of life issues, family,
emotional support
Your pt is sunk. won’t be getting any better, if anything, progressively worse no matter what meds are given. Better check on pt, might not be there much longer and you’ll have to do another care plan.
