one to one (observe at arm’s length), watch for harmful things in the client’s room, establish a contract with the client, ask if he has a plan…etc…I suggest you look into a book..it will give you rationales also…
Doing a care plan takes some thinking.
But they are really quiet easy.
I used to sweat over the darn things. Not anymore.
Break it ALL down into manageable pieces.
Pretend, because you have no ‘real’ patient in front of you, that YOU are the patient.
How would you be feeling?
How would you act?
How would you be treating your body?
What risk would you be?
What risks are there to you?
I can give you if you want the most helpful link to a fantastic Nursing site that helped me out so much when I was a student? And I have sent many other people there.
They have a permanent link to care plans.
