you can check out NANDA online for some ideas
First I would put
1. alteration in cardiac output
2. alteration in comfort: pain
3. then something else related to the patient. perhaps non compliance, alter in nutrition or any number r/t the patient
Pain. Related to: history of heart disease. AEB: patient complains of anginal pain. Goal: patient shall remain pain free and voice complaints of pain to staff immediately.
Excercise intolerance Related to: history of heart disease. AEB: patient unable to ambulate more than 20feet without feeling SOB. Goal: patient shall gradually increase distance of ambulation without dyspnea.
Dyspnea Related to: heart disease AEB: patient become easily fatigued and shsort of breath upon exertion Goal: patient shall gradually increase activity until she can further tolerate more exercise and activities.
High risk for falls Related to: History of HTN AEB: patient complains of vertigo when blood pressure is elevated. GOAL: patient shall use call light and ask for assistance when getting out of bed, chair, commode.
Please look this up yourself and do your own homework! You are going to be responsible for real people with this and other illnesses. I see medical people asking all the time on here for help cheating on their homework and it really scares me!
Think of what this illness does to the body and understand what help your patient will need!
