A care plan is almost always done with the client, or with clients Power of Attorney, All information given at time of the assement must be as close as practical to correct, otherwise, without knowing full details of that persons medical needs, appropraite care cannot be given.

Our nursing care plans are done with the client OR with the POA of the client…this way we know what they would also like for their family member to be doing and what they should not be doing. We give out input of what we feel is right, but we can never go against what the client or the family wants. In the event that there is no POA, and the client is not alert and oriented, then usually it is taken into the hands of the nursing staff to do what they feel is the right thing to do in order to allow the client to get better fast, but without too much force.

should be with, although I know some of the time it is easier for the treatment team to do it without the client and “review” it w/them later (tisk tisk…not quite client centered if you ask me).
