You need general objectives, then you can make up specific interventions for each.
For example:
Ensure proper healing of the perineum (tear or episio);
Patient will bathe the area twice a day in luke-warm water.
Nurse will visually monitor swelling and signs of infection once a day for the first three days.
Ensure proper uterine descent;
Nurse will verify the uterus level (stage) every eight hours until below pelvic bone.
Ensure proper nutrition for the newborn;
Nurse will teach proper latch-on techniques.
Patient will demonstrate adequate breast-feeding/ bottle-feeding knowledge before being released.
Nurse will weigh child every day to monitor growth.
etc etc etc.
You can talk also about offering psychological support, proper care of the newborn, making sure child leaves secure in a regulation car-seat, offering support to the new Dad, pain-relieving (especially C-section) etc etc
