I need to do a care plan on a client who just had an IVC filter surgically inserted. The client is also suffering from acute pain (she has diabetic neuropathy and pain from her surgical incision). Do you think the best nursing diagnosis would be risk for infection R/T surgical incision? Or am I completely off?
Also, the client is suffering from constipation and fatigue…

As an RN with 20 years of experience I could go in many different directions with this question…lol. I would say one of several thing. According to NANDA Guidlines, care plan diagnosis have to be written in a certain way. Here are some suggestions for you.
1) Potential for infection related to recent surgical procedure
2.) Alteration in comfort related to patient complaints of constipation and past diagnosis of neuropathy.
3.) Alteration in comfort related to recent surgical procedure.
4.) Potential for alteration in respiratory/airway status secondary to recent IVC filter placement.
