A care plan is what a blue print is to an architect. It maps eveything out for us. It is a plan that all the nurses taking care of someone follows. It tells us what to work on, what we want to accomplish, how to work on it, and how it went. For example- a paralysed patient – a nursing diagnosis could be – risk for skin impairment , due to immobility. the goal would be – pt’s skin remains intact without redness. We imply implements – 1) turn pt every two hrs 2) pad bony areas 3)ensure proper nutrition 4) check skin every shift. then we evaluate how each of the implements went. A care plan is something we create to address all the other things that are a problem or could be, that aren’t an actual medical diagnosis. I don’t know if that answers your question.
